The Empowered Endings approach to End of Life Planning goes beyond the standard medical model’s version. In each session, a physician will do an extensive exploration of your life experiences, goals, wishes, resources, needs, and interests; facilitate communication with and understanding of family members regarding your wishes; and complete the necessary documentation through our proprietary HIPAA-compliant process that results in a highly detailed Advance Health Care Directive (AHCD) and videos articulating and confirming the documented wishes.

These documents are safely stored and easily accessible via a unique QR code to ensure family members and healthcare professionals can access them in an emergency from any location without having to keep the hardcopy paperwork in hand. For example: In the event of a medical emergency during a vacation, the treating provider(s) / facility can scan the QR code and receive a copy of the patient’s healthcare wishes. Because we offer planning nationally, our advance directive is valid in all 50 states.

The current medical model’s version of End of Life Planning is a 15-minute visit with your physician covered by insurance. It is our experience, and many others’, that this is woefully inadequate.

We offer a complete advance directive that is stored on a secure HIPAA compliant platform and guided by a physician in a 60-90 minute session, plus a supported follow-up conversation with your Health Care Agent and/or family for a one-time fee of $2,499 or 3 installments of $999.
